Install Forms add-in to PowerPoint to create and insert forms or quizzes into your presentation. Forms add-in may already be installed- check your insert tab to confirm. If not, please follow the installation steps:
- Open PowerPoint and open a blank document.
- Click File > Get Add-ins.

- Select Admin Managed, then double click Forms.

- This will add Forms to your "Insert" bar.

- Open PowerPoint and select the slide where you want to insert the form.
- Select the Insert tab.
- Select Forms > Add.
- If the Forms add-in isn't available- select Get Add-ins > Admin Managed, then Refresh.
- Create a form in Microsoft Forms.
- Copy the link to the form.
- In PowerPoint, select Forms from the "My Add-ins" drop-down menu.
- Paste the form URL into the slide and select Insert.
