Summary

This article defines the procedure for reporting issues with Hearform, an Office Management Software (OMS) focused on Audiology, utilized by the Callier Center.

Procedure

  1. Verify that restarting the computer does not resolve the issue.
  2. Navigate to the Endpoint Software Support service page.
  3. Select the Report Issue on a UTD Device service offering.
  4. In the Description, note down the below:
    • The issue
    • Any errors present (if possible, include screenshots)
    • Device asset number 
  5. Click Submit.
  6. Callier Center technicians will reach out to resolve the issue.
