To Request a New Teams Voice Number / Extension for an Individual

  1. Open the “Request New Individual Extension” form.

  2. Add “Additional Contacts” to the ticket (such as a supervisor), it’ll notify individuals other than the requester when the ticket is updated.

  3. Add a Subject as “Teams Voice New Hire Activation Request”.

  4. In the Description field, state your activation request. Does the new user need to have their line masked for external calls, if so please include that information

  5. For the person receiving the Extension, fill out their NetID, Name, and their Department.

  6. Enter the name of the person receiving the extension under the Caller ID Display.

  7. Check the appropriate box if they require outgoing external calls to be masked.

  8. Check the appropriate box if availability of a specific extension needs to be checked.

  9. Check the appropriate box if they require International Calling.

  10. Choose Requested Completion Date, the deadline by which you'd like the request to be completed.

Note: We are only able to assign campus extensions to current Staff, Faculty, TA's, and RA's. Retiree's are not eligible due to our licensing agreement with Microsoft.
Note: If an employee is moving to another department and assuming the extension of an employee that is leaving, please use the Transfer Extension Form instead.

 

To Transfer / Remove Individual Extensions 

  1. Open the “Transfer/Remove Individual Extensions” form.

  2. Add “Additional Contacts” to the ticket, it’ll notify individuals other than the requester when the ticket is updated.

  3. Add a Subject, Description, or any Attachment(s) to the ticket.

  4. Select the option to Transfer, Modify, or Remove a person’s extension as per your request.

  5. Enter the Name/ NetID of the Person Losing Extension & the Person Receiving Extension (If Transfer is selected on Step 4).

  6. Type “Yes” under Confirm Extension to Transfer/ Remove.

  7. You can write the name of the person receiving the extension under the Caller ID Display.

  8. Check if you will require International Calling with your extension.

  9. Choose Requested Completion Date, the deadline by which you'd like the request to be completed.

Note: You will automatically receive email notifications when this ticket is updated.
