Instructions for Liquid Waste
Fill in the makeup of the waste, along with estimated VOLUME / MASS and UNITS in the corresponding columns. DO NOT use percentages. This section can also be used for primarily liquid wastes with small amounts of solids dissolved or settled.
Please do not use formulas or abbreviations when describing chemicals.
- EXAMPLES: Use "Acetone" instead of "(CH3)2CO" and use "Dichloromethane" instead of "DCM."
- You must provide more detail than just “Organic Solvents,” "Chemically Contaminated Water" or similar. The specific solvents, contaminates, and similar must be listed under the constituents. "Trace" can be used for a small but unknown amount.
- EXAMPLES: Use "Water, 3 L; Chromic Acid, trace" instead of "Chemically Contaminated Water" or "Chloroform, 14 L; Dichloromethane, 6L," instead of "Organic Solvents."

Instructions for Solid Waste
Fill in the makeup of the waste, along with with the either the MASS or PERCENTAGE (%) and UNITS in the corresponding columns.
Please do not use formulas or abbreviations when describing chemicals.
- EXAMPLES: Use "Acetone" instead of "(CH3)2CO" and use "Dichloromethane" instead of "DCM."
- You must provide more detail than just “Contaminated Solids,” "Used Silica," "Heavy Metals" or similar. The contaminates must also be listed under the constituents. "Trace" can be used for a small but unknown amount.
- EXAMPLES: Use "Filter paper, paper towels, gloves, 100%; Magnesium Sulfate, Trace" instead of "Chemically contaminated solids."
Instructions for Gaseous Waste
Please provide the percent makeup or "balance" of the gas. DO NOT use formulas or abbreviations when describing chemicals
- EXAMPLE: Use "Propane" instead of "C3H8" or "Trimethylaluminum" instead of "TMA."
