  • Purpose: Used to indicate the employee’s tax withholding allowance (not required to fill out each year).

1098-T

  • Purpose: Used if your higher education expenses may be eligible for federal tax benefit.

1042-S

  • Purpose: Used for annual statement regarding scholarship and assistantship income for foreign students.

1095-C

  • Purpose: Used by Employees who worked an average of 30 or more hours per week or enrolled in the health insurance plan through UT Dallas at any time to prove insurance coverage status.
