How to Integrate MS Forms with Teams

Add a Forms Tab to Create a New Form

  • Open Teams and click on the Chat or Channel you would like to add an MS Form to.

Plus sign icon located on top panel of selected chat/channel.

  • In the pop-up window, you can search for "Forms" and select the Forms icon.

Create New Form

  • After selecting Forms App to be added, click on Create a shared form... > Save.

Note: "Post to the channel about this tab" is automatically checked so you can immediately share your new form with your team. If you do not want to share it right away, deselect this option.
  • Each form tab will have an action status in front of the form title.
    • In the following example, this Test Form is in the process of being created, so "Edit" is in front of the form title: Edit | Test.

Add Existing Form

  • After selecting Forms App to be added, click on Add an existing form.
  • Select the Form you would like to add, then choose whether to Collect responsesShow results, or Collaborate from the drop-down menu.

  • If you select Collect responses, the form will be in "Fill" mode.
    • You'll see "Fill" in front of the title to let you know you're in the process of collecting responses from your team. Example: Fill | Test.

  • If you select Show Results (sharing Form results with team), the form will be in "Results" mode.
    • You'll see "Results" in front of the title to let you know responses will be collected and shared with your team. Example: Results | Test.

  • If you select Collaborate, you and your team can edit an existing form. 
    • You'll see "Edit" in front of the title to let you know the form is open for edit. Example: Edit | Test.

Form Conversations

  • Now that you've created a new form or added an existing one, anyone on your team can contribute to your form. It's easy to get a conversation going about it by clicking on the  Conversation icon on the upper right corner of Teams.
    • A conversation window will open to the right of your form.
