Microsoft Forms

What Is It?

With Microsoft Forms, you can create surveys, quizzes, and polls and easily see results as they come in. Microsoft Forms should not be used for instructional purposes when a grade is assigned (quizzes/tests). Please use eLearning for this purpose.

Where Can I Get It?

To access Microsoft Forms, visit forms.microsoft.com, and log in with your NetID@utdallas.edu and normal UTD password.

How Much Does It Cost?

There is no cost for this service at this time.
